The U.S. Food and Drug Administration (FDA) issued a new warning on Friday regarding the popular painkiller fentanyl, basically a re-issue of its 2005 warning. The FDA estimates that the patch version of the potent painkiller is suspected in more than 3,500 deaths.

Fentanyl patches (brand name Duragesic) are typically prescribed to patients with chronic pain, or to those who have trouble swallowing pills. The patch's transdermal system releases a uniform dosage of the drug over the course of three days.

“I think there is more that needs to be done, or else we’re just going to see this [warning] happen again another two years from now.” “The deaths are going to continue.”

The FDA also warns that heat and exercise can cause the patches to release the drug at a faster and potentially lethal rate. Patients who experience breathing problems while using fentanyl should call their doctors immediately, because high doses of the drug can cause the patients respiratory system to fail.

And almost had me, but something came to mind when I read an article in the July 2, 2008 edition of the JAMA (Journal of the American Medical Association) by Dr. Michael Stillman. My friend presented me with the idea that general care physicians are not always very knowledgable about ailments or certain health problems that specialists would know. I had a personal experience that comes under this category. However, when the patient of a primary care physician is ill or injured, the primary care physician will know the patient well. One of the most important aspects of health care that is commonly overlooked is knowing the patient. Often such luxury is not possible with the busy schedule of doctors, or the lack of need to see certain doctors. Nonetheless, it can influence care dramatically. Dr. Stillman gave an example in his article where the medications of a patient had been changed to a more expensive regimen without his knowledge. The prescribing doctor would not have know that the patient could never afford those medications, but Dr. Stillman knew and prescribed the original, less-costly, and equally effective regimen.  A number of other issues come up as well when identifying the importance of primary care physicians. When making an important medical decision, trust between a patient and the doctor becomes essential. Patients, even if are under the care of a specialist may want to consult a phyiscian with whom they are familiar before making an important decision. Primary Care doctors may not necessarily save lifes, but they can certian help people maintain the highest quality of life possible. I witnessed something at work today which really made me mad -- and almost makes me want to call the Loss Prevention office.  One of my co-workers, we'll call him J, decided he would help his grandma by picking up her Lantus for her yesterday.  He came through the drive-thru and end of story, right?  No, he leaves work today, goes out to his car, and then minutes later comes back in with a prescription bag in his hand.  HE LEFT HER INSULIN IN HIS CAR THE ENTIRE DAY!  The high was 90 today!  He's such an idiot.  So does he refill the prescription, call her insurance for an override, and then pay another copay?  No.  He gets two new vials from the refrigerator and takes them home!  For those of you playing along, that is essentially robbing the store of $218.99.  That is a direct cash loss because you're stupid and think you can leave your drugs in the car like a library book or a movie from Blockbuster.  My question to you is:  would you have left your double cheeseburger that you got from McDonald's in the car?  (He's overweight, so that joke is kind of lost if you don't know him.)

Okay, so some people in the crowd want me to give him the benefit of the doubt.  "Whatever, it was a silly mistake and you should just make fun of him once he leaves the store."  No, I can't believe he didn't pay for a refill!  Can you imagine walking back into McDonald's after leaving your double cheeseburger in the car overnight and asking them to exchange it for a new fresh one?  They would laugh in your face!  (And a double cheeseburger only costs $1.  It's even more preposterous to ask them to exchange 219 double cheeseburgers!)  But golly, here at the drugstore, it's not your fault that you're not responsible.  We're just like a department store with a great return policy.  Except for the fact that by law we can't dispense drugs which have already left the pharmacy.  But that's not your fault, I guess.  Are we not teaching personal responsibility anymore?

Stalkers. Creepy, weird, and generally socially inept with a bit of malfunctioning chemical hard-wire.

After I graduated from high school, I moved to the nearest city and lived and worked for a year before deciding to go to university (something I highly suggest if you are broke and really don't know what you want to do with your life).

The day after I moved to the city, I started my new job, working in a pharmacy, in the back where the action happens: the drug dispensary.

I have maaaannny stories about working in pharmacies (I worked as a Tech for 5 years), some funny (I took a call once, where the guy on the other end told me seriously without laughing, that his penis was too big for even the Magnum sized connies and what should he do - I was laughing too hard and had t pass it to the pharmacist), gross ("Pharmacist, I haven't taken a dump in a week, what can I take?), NSFW gross ("Um, so I have this strange fluid leaking from my anus..." - I'm not even kidding people, if you want to go into pharmacy, have an iron stomach because people come in there as if they are talking to their doctor, no one hides anything). There was also the d-bag who would hit on me everytime he came in to fill his Valtrex (herpes medicine) as if I didn't know what it was for. Ew. Ew. No.

But there is only one creepy story.

About two months after I was hired, I got a strange call at work.

"Hello", I answered.

"Um, hi, you don't know me, but I know you [I swear to god, he said this]. I have been watching you" [ack, creep alert!], he told me.

"Who is this? Tom if this is you, I swear to Lucifer, I will melt down your motherboard when I get home!" I spat into the phone. (At the time, I was living with 3 guys, one of whom was my boyfriend at the time. Yeah, I know how crazy this sounds. They were messy and dirty, but that's another post).

"Oh, uh, no, no, you have never seen me in person, I come in to fill my prescription and I have seen you a couple times and you look like an angel. I was wondering if you were interested in being my friend?"

"Uhhhh, that's not a good idea, I am flattered but, uhhhhhhh, I have to go." And I quickly made "busy" noises (slamming things, tapping on the computer keys) before hurriedly hanging up the phone. [SHUDDER]

I was thoroughly freaked out by the fact that he had been watching me, looking, staring, observing from afar, yet I didn't know what he looked like, except that he had the stalker-talk down to a movie script finesse. 'You don't know me, but I know you' and 'I've been watching you' - dear.god

I told my boss and my co-workers about it and they told me not to walk home alone (which I did because I lived close by), and to think about getting a cellphone (which I did not, until my mom threatened my life until I bought one so she could talk to me every night for a month while I walked home after work).

It didn`t end there.

No, he called the next day, and the next, until after losing sleep for a couple nights, I got on the phone and told him that no, I didn`t want to be friends, that his whole approach had made me uneasy and that should he call me again, I would contact the police and set about getting a restraining order.

He quietly said that he understood, which made me feel really guilty, but at the same time, I felt scared and stressed out, and no friendship or relationship should ever be founded on either of those things. So he left me alone and the calls stopped, though to this day, I never found out what he looked like, never met him in person, and completely forgot all about him.

Until the scary Tylenol 3 addict who forged doctors prescriptions, showed up and refused to leave the store, making my stalker seem friendly and harmless in comparison. Stay tuned for more pharmacy phun.

304 out-patients with non-psychotic major depressive disorder (clinically established diagnosis verified through DSM-IV criteria) enrolled in the Sequenced Treatment Alternatives to Relieve Depression (STAR*D) study who did not achieve remission (remission defined as Quick Inventory of Depressive Symptomatology score 5) with first-line citalopram treatment. This analysis included only those people who agreed to be randomised to treatment strategies (augmentations, switches or both) which would allow cognitive therapy to be compared with pharmacotherapy.

Used one Speedotron strobe in a soft box for illumination. Props are on a hand painted canvas. The original film was hand developed in Rodinal and custom printed on fiber based paper and given to the client to scan. 

I did do an interesting experiment to prepare the image for this blog. Instead of scanning the 4x5 negative I shot it with a Nikon D2x and 55 micro lens on top of a lightbox. Imported the image into Photoshop and inverted the negative image into a positive one. Was very surprised at how well it came out and there was an added bonus, normally when scanning you get a lot of dust that has to be cloned out but this way it does not show up.

]]> <![CDATA[Always count your solid prescription pills or capsules]]> http://justaddh2o.wordpress.com/?p=16 Mon, 14 Jul 2008 19:07:44 +0000 justaddh2o http://justaddh2o.wordpress.com/?p=16 My mother is on a fixed income and on Medicare Part D so when she started calling anyone of her children including me to help her out on her prescription drug refill all siblings started to wonder why she needed refills too soon (3 days too early).  Some of her prescription was coming 1 to 3 pills short how often this had occurred none of us are sure.  So I called her pharmacist and they informed us that all solid prescription pills and  capsules are counted by machine by weight and they assured us that they will begin dispensing solid prescription by hand. (I believe they gave me this reason to just appease me.)   So my best advice, advocate for your self, your friend or your relatives that they count their medicines.  Drugstore pharmacies will not compensate the client for their inaccuracies.
